Job summary

Berkshire Healthcare NHS Foundation Trust is seeking a Nurse for Children and Young People in Care for 24 hours per week at Erlegh House, University of Reading. The role focuses on health assessments, care planning, health promotion, and multi‑agency collaboration to improve outcomes for looked after children in Berkshire.

Applicants should have experience with vulnerable children, be able to engage with young people in care, and be willing to travel within Berkshire and the 20‑mile radius.

Qualifications

• Experience of working with vulnerable children and young people.
• Ability to engage with children and young people in care.
• Willingness to travel within Berkshire and the 20‑mile radius.
• Enthusiasm for teaching and sharing knowledge and experience.

Responsibilities

• Undertake health assessments for children and young people in care aged 0–17.
• Assess and provide recommendations on plans of care.
• Provide health promotion advice on sexual health, contraception, smoking, diet, exercise.
• Manage a caseload as delegated by the Specialist Nurse.
• Participate in wider team training with the lead specialist nurse.
• Undertake audit, interpret data and use findings to improve practice.
• Contribute to inter-agency assessments and risk analysis.
• Attend relevant meetings (statutory reviews, professionals' meetings).
• Engage children and young people in feedback to inform service development.
• Collaborate with social care to implement health recommendations.
• Contribute to achievement of key performance indicators.
• Develop and maintain relationships with multi-agency partners.
