Job summary

Nottinghamshire Healthcare NHS Foundation Trust is seeking a Qualified Children's Wellbeing Practitioner to join the Citywide CAMHS team in the community. The role involves delivering brief, outcome-focused interventions for children and young people with mild to moderate mental health needs, in partnership with social care, health and education colleagues.

You will maintain high standards of safeguarding, contribute to care planning, and work across agencies to improve access to mental health

Qualifications

• Multidisciplinary collaboration with social care, health and education partners.
• Deliver brief, outcome-focused, evidence-based interventions for children and young people.
• Maintain confidential, high-quality case records and reports.

Responsibilities

• Assess and deliver brief, evidence-based interventions for mild to moderate mental health difficulties in children and young people.
• Work within a multi-agency team to support children, young people and families.
• Co-produce care plans with children, young people and carers where appropriate.
• Attend multi-disciplinary meetings and provide consultation across partner agencies.
