Job summary

Qualifications

• A recognized Social Work qualification (Degree in Social Work, DipSW, or equivalent).
• Registration with Social Work England is required.
• Experience working with children with disabilities and their families.
• Strong communication and interpersonal skills.
• Ability to work effectively within a team and independently, with good organizational skills.

Responsibilities

• Conduct assessments to identify the needs of children with disabilities and their families.
• Develop and review care plans in partnership with families and relevant stakeholders.
• Provide direct support and guidance to families, ensuring access to resources and services.
• Maintain accurate and timely case records and documentation.
• Participate in multi-agency meetings and safeguarding initiatives.
