Job summary

NHS South Yorkshire Integrated Care Board is seeking a Children\'s Continuing Care Team Leader to coordinate care for children and young people with highly complex needs. You will lead a multidisciplinary team, oversee assessments, and ensure services meet statutory requirements across South Yorkshire.

The role requires nursing or professional registration, degree-level education and a post-graduate management qualification or equivalent experience.

Qualifications

• Nurse or professional registration with relevant professional body.
• Degree-level education or equivalent experience in a specialist area.
• Post-graduate management diploma or equivalent, or significant experience of it.

Responsibilities

• Provide operational and professional leadership to Children's Continuing Care staff.
• Support the assessment, review and commissioning of care packages for children and young people with complex health needs.
• Ensure compliance with the National Framework for Children and Young People\'s Continuing Care and associated NHS policies.
• Manage staff performance, wellbeing, supervision and development.
• Provide expert advice on complex case management and eligibility processes.
• Lead quality assurance, governance, risk management and service improvement.
• Develop partnerships with local authorities, education, acute and community services and independent sector providers.
• Support service transformation and continuous improvement across the All Age Continuing Care Service.
• Contribute to workforce planning, recruitment and staff development.
• Work alongside the existing team leader to provide day-to-day operational leadership to the case management team.
• Ensure services are safe, effective and responsive, in line with statutory requirements.
• Work closely with families, providers, education and social care colleagues and wider multidisciplinary teams
