East Suffolk and North Essex NHS Foundation Trust is recruiting a Children’s Community Nurse on a fixed-term/secondment basis to cover maternity leave until 1 November 2027. The role covers North East Essex and collaborates with Colchester hospital to enable care closer to home for children.

We seek a flexible, proactive nurse who can work independently within community settings and contribute to service development.

Responsibilities

• Manage own caseload of children in community settings, providing evidence-based nursing care.
• Perform clinical assessments, care planning, delivery and evaluation of nursing interventions.
• Collaborate with multi-disciplinary teams and Colchester hospital colleagues to support early discharge and care closer to home.
• Use autonomous judgement to identify acute changes and initiate appropriate nursing actions.
• Develop and implement care programmes to promote health gains and independence across home/school settings.

Job description

An exciting opportunity has arisen to join an established team of children’s community nurses on a fixed term/secondment basis to cover maternity leave until 1st November 2027. We are a busy service with a diverse caseload working across North East Essex. We provide clinical interventions to children within their own homes 7 days per week.

We are looking for a confident innovative nurse to join our children’s community nursing team. You will be required to work in conjunction with the Children’s department at Colchester hospital whose aim is to facilitate early discharge and care closer to home.

You will need to be a flexible practitioner within the community setting, a good team player and be able to develop collaborative working relationships with other healthcare professionals, services and agencies.

The post holder will be responsible for a caseload of children delivering high quality patient care according to agreed standards which are set and monitored.

You will be offered opportunities to develop both clinically and professionally within a supportive environment and encouraged to participate in service developments. Clinical supervision and a supported induction period are provided

• The post holder will be responsible for the management of their own caseload.
• To provide the delivery of evidence based nursing care to children the community settings by providing clinical assessment, care planning, care delivery, care evaluation and clinical advice in accordance with the NMC Code of Professional Conduct.
• To develop planned programmes of care to promote health gains and maximise independence in all types of children within the community (for example, home, schools or other establishments) as required
• Using autonomous judgement and liaising with the Matron and/ or multi-disciplinary professional as appropriate, manage children’s health needs in accordance with clinical guidelines, using own clinical judgement to recognise acute exacerbations of illness and to initiate appropriate nursing interventions.
• Using autonomous judgement and liaising with the Matron and/ or multi-disciplinary professional as appropriate, manage a changing caseload of children and identifying when the child’s condition warrants discharge from the service.
• To implement and evaluate evidence based training programmes for families, carers, other professionals and the multi disciplinary team.
