Job summary

South Gloucestershire Council is seeking a Social Work Assistant to work with disabled children and young people, their families and wider networks, providing practical support to promote inclusion and independence.

You will be part of the 0-25 Social Care Service within the 0-18 team, coordinating support and ensuring voice of the child remains central in decision-making and practice.

Qualifications

• NVQ Level 3 or equivalent, or working towards a qualification related to services for children and young people.
• Experience of working with children, young people and families in a statutory setting.
• Experience or knowledge of risk assessment and risk management.
• Knowledge of childcare legislation, including child protection, regulations and codes of practice.

Responsibilities

• Organise and chair review meetings, prepare and share reports, and represent the team at multi-agency meetings.
• Visit children and young people in homes and schools to build positive relationships and understand lived experiences.
• Work with agencies, providers and families to ensure coordinated support
• Review plans created by social workers to ensure progress and positive outcomes.
• Gather and share information to inform reviews, assessments and plans with the child or young person at the centre.
• Contribute to Education, Health and Care Plans (EHCPs) with professionals to support outcomes.
