Qualifications

• Nurse with autonomous decision-making and clinical judgment.
• Experience in risk assessment and risk management.
• Ability to support discharges and develop care plans with patients.
• Collaborative working with the multi-disciplinary team (MDT).

Responsibilities

• Lead and support junior staff to deliver high-quality nursing care.
• Coordinate and evaluate patient-centred care with the MDT.
• Administer and monitor medications; ensure safe storage and disposal of medicines.
• Undertake risk assessments and contribute to safety and escalation processes.
