Job description

We're looking for a dedicated Casework Coordinator to join our team in Bath, supporting individuals in supported housing who face complex challenges including homelessness, offending, and social exclusion.

Key Responsibilities

Manage and track referrals and client outcomes

Complete needs and risk assessments, creating tailored support plans

Help clients understand their rights and responsibilities upon entering the service

Work closely with agencies like Probation, Police, and support services

Take part in the on-call rota (out-of-hours)

Experience working with people with complex needs or behaviours

Understanding of the causes of offending and effective support strategies

Strong interpersonal and coordination skills
