Job summary

Horizon Health Services seeks a Case Manager Supervisor to lead TCM and PRS teams in a dynamic behavioral health setting. You will coach staff, oversee referrals, ensure compliance with Medicaid and state standards, and drive quality assurance through documentation reviews.

Split between two NY locations (Lockport and Niagara Falls), the role requires strong leadership and collaboration with internal and community partners to sustain effective care delivery and program outcomes.

Qualifications

  • Bachelor's degree in Social Work, Human Services, Psychology, Rehabilitation Counseling, or related field is required.
  • 2 years of experience in behavioral health and vocational work is required.
  • 2 years of supervisory or leadership experience is required.
  • Knowledge of behavioral health, recovery-oriented care, and trauma-informed practices.
  • Understanding of Medicaid-funded services and community support systems.

Responsibilities

  • Bachelor's degree in Social Work, Human Services, Psychology, Rehabilitation Counseling, or a related field required.

Location:

This position will be split between two locations: 637 Davison Road, Lockport NY and 2400 Pine Ave, Niagara Falls NY. Travel between the two sites may be required.

Hours:

Monday - Friday: 8am - 5pm

Compensation:

Horizon is consistently evaluating our compensation and benefit packages to ensure we remain competitive. This position starts at a salary of $64,000-$66,000 annually. Starting pay may be impacted based on experience and education. Compensation adjustments are evaluated annually.

This position entails eligibility for a sign-on bonus of up to $1,500 for your commitment to work at our Horizon Certified Behavioral Health Clinics (CCBHC) located at our Pine Avenue Recovery Center and Lockport Recovery Centers for at least one year. A change in scheduled hours or location of employment may impact eligibility of bonus.

Physical demands associated with this position include:
  • Sitting for extended periods, alternating between standing and sitting
  • Mobility required with repetitive wrist, hand, and finger movements
  • Handling light duties, occasionally lifting objects up to 20 pounds
  • Exposure to various environmental conditions including low/high temperatures, outdoor elements
