Job summary

Cardiff Council is seeking a Carers Assessment Worker within Independent Living Services to support unpaid carers by providing tailored information and assistance, linking to Health & Social Care and referring to other agencies.

The successful candidate will have excellent communication and IT skills and experience in social care or related environments, with a strong focus on resources management and customer outcomes. Welsh language ability would be advantageous.

Qualifications

• Excellent communication and IT skills.
• Experience of working within a Social Care, Health, Third Sector or advice giving environment.
• Strong track record of managing resources effectively and providing excellent outcomes for customers.
• Ability to respond sensitively to individuals’ circumstances.
• Welsh language ability would be an advantage.

Responsibilities

• Provide advice, information and assistance to unpaid carers, linking into Health & Social Care Services and referring to other agencies.
• Work as a member of the Carers Assessment Team to support carers and coordinate with partner services.
