Job summary

Cygnet in Markfield (Woodrowe House) is seeking a compassionate Care Rehabilitation Assistant to support residents in recovery and daily activities within care home or home care settings.

You will follow care plans, assist with personal activities, maintain a clean environment, support meals, communicate with the team, and uphold confidentiality. DBS check is required and training is provided.

Responsibilities

• Support residents with personal activities (washing, dressing, toileting) as per care plans and goals.
• Encourage participation in activities aligned with residents' wishes.
• Maintain a tidy and clean environment.
• Assist with meals and hydration as per care plans.
• Support effective team communication and report issues to senior staff.
• Maintain confidentiality and data protection in all aspects of the home.
• Participate in mandatory training and adhere to home policies.
• Keep up-to-date knowledge of resident care plans.
• Recognise and escalate risks to resident safety to senior staff.
• Ensure documentation is accurate and timely.

We are seeking a compassionate and dedicated Care Rehabilitation Assistant to join our team. This role involves supporting individuals in their recovery and daily activities within care home or home care settings at Woodrowe House.

Woodrowe House is a leading neurological complex care service located in the picturesque outskirts of Leicester, in Markfield. We specialise in complex care and treatment for individuals with acquired brain injuries (ABI) and other neurological conditions, including stroke and spinal injuries. Our mission is to provide a supportive and expert environment for long-term and short-term care needs.

We work on a 2-week rolling shift pattern, consisting of a short week working Wednesday and Thursday 08:00am-20:00pm and a long week working Monday, Tuesday, Friday, Saturday & Sunday 08:00am-20:00pm.

Your day-to-day…

• To support and enable residents with personal activities (such as washing/bathing, dressing, toileting) as part of their day to day care and in line with written care plans, personal goals and wishes.
• To be driven by resident’s needs and wishes in supporting and enabling their participation in activities and pursuits of their choice.
• Maintain a tidy and clean environment
• To support residents, as per their care plans, to prepare, eat/drink, clear away meals, snacks and drinks.
• Help to ensure effective communication throughout the team, by participating in meetings and bringing to the attention of a senior staff member anything that may have significant effect on the well-being of residents.
• To ensure confidentiality and data protection is maintained in all respects whether concerning, residents, staff members or the business of the home.
• Participate in training events, some of which are mandatory.
• To adhere to all policies and procedures within the home.
• Maintain working knowledge of resident care plans.
• Recognise the need for action in any event where a resident may be at risk of harm and raise the matter immediately with a senior member of staff.
• Comply with the home’s requirements regarding effective documentation.
