Service Care Solutions is seeking an experienced Care Navigator to support GP practices and Primary Care Hubs across Sefton and the wider Liverpool area.

The role involves connecting patients with GPs and healthcare professionals, providing information about services, and ensuring a smooth patient journey. Flexible on-going hours (part-time/full-time) and location across Liverpool are offered.

Formación

• Contract-based opportunity within primary care.
• Previous experience as a Care Navigator or Receptionist within a GP practice, health centre, or primary care setting.
• Experience using EMIS is desirable.
• Excellent communication and interpersonal skills.
• Strong organisational and administrative abilities.
• A patient-focused approach with the ability to handle enquiries professionally.
• Ability to work effectively with healthcare professionals and administrative teams.
• Provide administrative support to GP practices and Primary Care Hubs.
• Handle patient enquiries and provide information about healthcare services and procedures.
• Work closely with GPs and other healthcare professionals to support effective patient care.
• Maintain accurate patient records and databases using the EMIS clinical system.
• Carry out patient navigation duties, directing patients to appropriate services and support.
• Help ensure a smooth and efficient patient experience.
• Support the day-to-day administrative operations of primary care services.
