The Pcn is seeking a care navigation professional to meet patients or carers in convenient locations and to support them in coordinating access to NHS and community services.
You will identify needs, arrange referrals, and act as a link between primary and secondary care within the PCN to ensure joined-up, person-centred support for patients and carers.

Confidentiality The post holder must maintain the confidentiality of information about patients staff and Health Service business in accordance with the Data Protection Act (1984).

To promote a service culture in which clinical governance becomes an integral part of normal working and to ensure that the requirements of clinical governance are met by any staff for whom the post holder is responsible.
