An exciting opportunity has arisen to join the Community Rehabilitation Team as a Care Manager within Adult Social Care. This role is offered on a part‑time basis, with a commitment of 27 hours per week. As a Care Manager, you will collaborate closely with health and social care partners to support individuals living with significant mental health needs within the community. Your work will focus on enabling recovery, fostering independence, and facilitating safe discharge planning where appropriate.
Interview Date: 10th February 2026. Please note, this role is not eligible for visa sponsorship and we can only consider applicants with an existing right to work in the UK.
