Job summary

The Northern Health & Social Care Trust is seeking a Care Management Support Worker (Band 4) for the Community Mental Health Teams in Moyle, with initial location at Larne CMHT in Moyle Medical Building.

You will secure and monitor support services for individuals open to CMHTs, carry out a Key Worker role as required, and contribute to care planning and review processes with experienced staff.

Qualifications

• QCF Level II in Care or equivalent, willing to undertake and obtain QCF Level III within 4 years
• Two GCSEs (A-C), including English, and experience in caring for individuals with functional mental illness
• Eligible for NISCC Register of Social Care Workers; valid driving licence preferred and access to car

Responsibilities

• Procure and secure services (Packages of care, Direct Payments, Short Breaks)
• Disseminate information to colleagues (reviews, waiting lists)
• Support carers and identify needs via carer assessments
• Coordinate Short Breaks / Placements
• Contribute to person-centred care plans under supervision of Band 6/7 staff
• Maintain records and participate in supervision and meetings
• Participate in MDT working and extend provider networks to support independence
