Job summary

Civic Recruitment Limited is recruiting for a 3 month contract with a local authority to support assessment, care management and the coordination of services for vulnerable adults and their carers. The role focuses on promoting independence, choice, and wellbeing through person-centred approaches.

The post requires experience in social care settings, strong communication skills, and the ability to work within a multidisciplinary team.

Qualifications

• Experience working with vulnerable adults in health, social care or related settings.
• Understanding person-centred approaches and promoting independence.
• Experience contributing to assessments, support planning or care management.
• Ability to communicate sensitively with citizens, families and professionals.
• Willingness to undertake training related to assistive equipment and adaptations.

Responsibilities

• Contribute to assessing the needs of vulnerable adults and carers.
• Assist in effective support planning to maximise independence.
• Support health and wellbeing with a person-centred approach.
• Undertake care management duties and monitor support arrangements.
• Collaborate with multidisciplinary teams and community services.
