Job summary

Central London Community Healthcare NHS Trust is seeking a dedicated care professional to provide high-quality personal care and support to residents. You will lead and support care staff, ensure care plans are followed, and maintain accurate electronic records.

This role involves liaising with GPs, nurses and other professionals, responding to health changes, documenting daily care, and promoting dignity and independence. Competitive salary and pension scheme offered.

Responsibilities

• Providing high-quality personal care and support to residents.
• Leading and supporting care staff during the shift.
• Ensuring residents' care needs are met, and care plans are followed.
• Updating and maintaining residents' care plans and electronic care records accurately and regularly.
• Communicating effectively with GPs, nurses, district nurses, and other healthcare professionals.
• Responding appropriately to changes in residents' health and wellbeing and escalating concerns when necessary.
• Completing accurate care records and daily documentation.
• Supporting medication procedures in accordance with company policies and training.
• Communicating professionally with residents, relatives and healthcare professionals.
• Using your own initiative to deal with situations that arise during the shift.
• Promoting dignity, independence, choice and respect at all times.
• Supporting the smooth running of the care home and maintaining high standards of care.
