Job Description

We are seeking a dedicated Care Coordinator to join our supported living services in Reading. This role involves working closely with the residential manager to coordinate staff for two supported living services and one outreach client, supporting adults with learning disabilities, autism, and mental health needs.

Key Responsibilities:

  • Coordinate and schedule care and support for service users across supported living settings.
  • Ensure care plans are up to date and effectively implemented in collaboration with the Registered Manager.
  • Maintain accurate records, including risk assessments and support plans.
  • Assist with staff rotas, shift planning, and last-minute cover arrangements.
  • Act as a key contact for staff, service users, families, and external professionals.
  • Ensure compliance with CQC standards, safeguarding policies, and internal procedures.
  • Participate in on-call duties and step in when needed.
Requirements
  • Previous experience in a care or support setting (supported living or domiciliary care preferred).
  • Experience with rota and shift coordination preferred.
  • Strong communication, organizational, and problem-solving skills.
  • Good understanding of person-centred care and safeguarding.
  • Proficiency in care planning systems and IT.
  • A caring attitude and commitment to empowering individuals to live independently.
  • NVQ Level 3 in Health & Social Care (or working towards) is desirable.
