Overview
General Practice Alliance (GPA) are looking for a Care Coordinator/Phlebotomist to join our team on a full-time basis. This role combines safe, compassionate blood sample collection with proactive care coordination. You’ll be working closely with patients to ensure they feel supported, connected to the right services, and confident in managing their health and wellbeing. Please note: We are unable to offer visa sponsorship for this role.
Interview dates: Stage 1 – 13 October; Stage 2 – 17 October. Preferred Start Date: 1 December 2025.

Responsibilities
- Perform safe and compassionate blood sample collection in line with clinical procedures.
- Accurately label, store, and dispatch blood samples to appropriate laboratories.
- Document sample collection and care coordination activities in patient records.
- Review patient notes to identify unmet biopsychosocial needs or opportunities for further support.
- Engage with patients to coordinate follow-up actions.
- Act as a central point of contact for patients, promoting continuity of care.
- Support patients in preparing for and following up on clinical conversations.
- Proactively identify frequent attenders with complex needs and coordinate their care.
- Signpost and navigate patients to relevant health, social, and community support services.
- Triage incoming referrals and direct patients to appropriate personalised care services based on individual needs and preferences.
- Escalate anomalies or significant patient concerns to clinical staff.
Person Specification
Qualifications
- Essential: GCSE Level (5 GCSEs at grade C or equivalent, inc. English Language and Maths).
Experience
- Essential: Experience in primary care, community, or hospital settings.
- Essential: Trained and competent in phlebotomy, with experience in blood sample collection OR willing to complete phlebotomy training shortly after appointment.
- Desirable: Experience in a care coordinator role.
Skills and Knowledge
- Essential: Ability to use databases to review patient records, identify needs, update information, and coordinate tasks; commitment to confidentiality, safeguarding, and patient safety.
- Essential: Strong listening and empathetic, person-centred support; non-judgemental approach.
- Essential: Organised with the ability to manage multiple tasks and priorities.
- Essential: IT literacy (word processing, email, internet).
- Essential: Basic knowledge of long-term conditions and their impact on physical, emotional and social needs.
