We are looking for a new part-time Care Coordinator on a fixed-term contract to cover maternity leave. The candidate will have excellent communication and administrative skills to join our Ageing Well team working on the Enhanced Health in Care Homes and Housebound service.
You will work alongside GPs and other professionals supporting patients living in Care Homes.
Main duties of the job
- Work closely with GPs and primary care professionals within the PCN to identify and manage a caseload of patients, ensuring appropriate support is provided and their changing needs are addressed.

We are a Primary Care Network (PCN) with 9 member GP practices in Lincolnshire. A PCN is a group of GP practices working together to provide NHS services to our patients. IMP Healthcare provides services to over 70,000 patients, including Mental Health, Social Prescribing, Enhanced Health in Care Homes, Housebound services, clinical pharmacy, and physiotherapy. We also offer an Extended Access service outside of core GP hours.
Job responsibilities
- Support patients, families, and carers to understand their conditions.
- Develop and review personalized care and support plans.
- Coordinate and navigate services across health and care sectors.
- Collaborate with GPs and primary care professionals to manage caseloads and refer to other professionals when needed.
- Support multidisciplinary team delivery within the PCN.
- Promote shared decision-making and support staff and patients in these conversations.
- Identify and support individuals who could benefit from care coordination.
- Engage in empathetic conversations about needs.
- Develop knowledge of local health and care infrastructure.
- Support the development and implementation of personalized care plans.
- Review and update care plans regularly and communicate them to relevant professionals.
- Manage appointments and facilitate patient transitions between services.
- Refer patients to social prescribing and other support services.
- Liaise with multidisciplinary teams to ensure coordinated care.
- Participate in team meetings and identify additional support needs.
- Record interventions and patient progress.
- Engage in continuous professional development with supervision from GPs or ANPs.
- Build strong relationships within the practice and PCN team.
- Champion personalized care and shared decision-making.
- Perform other duties as required, ensuring timely and effective work delivery.
- Identify service improvement opportunities and contribute to policies on equality and reducing health inequalities.
- Adhere to organizational policies including confidentiality and safeguarding.
- Support the wider objectives of the PCN to improve primary care services.
Person Specification
Qualifications
- Good secondary education, including English and Maths.
Experience
- Experience in healthcare or public-facing roles with excellent customer service skills.
- Primary Care/General Practice experience is desirable.
- Good IT skills.
