Qualifications

  • Expérience en tant qu'infirmier en santé mentale, travailleur social ou thérapeute occupationnel.

Job description

We currently have several opportunities available for experienced Mental Health Nurses, Social Workers, and Occupational Therapists to work as Key Clinicians (formerly known as Care Co-ordinators) in our new Core Community Teams.

By joining our Mental Health Community Services, you’ll be working in a vibrant, diverse part of London—Haringey—famous for its excellent restaurants and numerous award-winning parks.

Recent investments in our services have expanded our capacity to support recovery through closer partnerships with the Voluntary Care Sector, Community Occupational Therapy, and physical health services. You will also have opportunities to develop your skills by working within integrated treatment pathways.

Main duties of the job
  1. Manage a caseload as a care co-ordinator, following the Dialog Plus (formerly CPA) framework.
  2. Deliver effective, multidisciplinary care and treatment in collaboration with other agencies.
  3. Engage service users and their carers in care planning, addressing their individual needs.
  4. Create personalized care plans to promote self-management and social inclusion, reviewing and updating them as needed.
  5. Provide evidence-based therapeutic interventions, utilizing skills such as CBT or psycho-education.
  6. Communicate clearly with service users and carers to support informed decision-making.
  7. Administer medication, monitoring effects and side-effects as per guidelines.

Flexibility in work location may be required, with possible placements across Barnet, Camden, Enfield, Haringey, or Islington. The Trust reserves the right to assign staff to other locations as needed.
