Job summary

CCH Group is seeking a dedicated Care Coordinator in Leeds to organise and oversee care workers and ensure high-quality community care. The role focuses on coordinating teams, reviewing care plans, and delivering exceptional service within a Monday–Friday, 9am–5pm framework.

You will assess and update care plans, supervise staff, and build strong working relationships with carers. This is a client-facing position with opportunities to contribute to service improvements and career progression.

Qualifications

• Experience in healthcare settings and CQC knowledge is beneficial.
• Strong organisational and rota management skills.
• Ability to build strong relationships with care workers and field teams.

Responsibilities

• Assess and review individual care plans to ensure quality of care.
• Supervise, instruct and support care staff to meet standards.
• Coordinate care delivery and optimise scheduling across locations.

What you’ll do

The Care Coordinator plays a key role in ensuring the highest quality care services by managing care workers. Responsibilities include assessing and reviewing individual care plans, as well as supervising, instructing, and supporting care staff.
