Job summary

CCH Group in Liverpool is hiring a Care Coordinator to help organise and deliver high-quality home care in the community. You will manage care workers, assess and review care plans, and provide supervision and support to staff to ensure excellent patient outcomes.

The role operates Monday to Friday, 9am–5pm, with opportunities for career growth through leadership development and access to expert guidance and innovative systems.

What you’ll do

The Care Coordinator plays a key role in ensuring the highest quality care services by managing care workers. Responsibilities include assessing and reviewing individual care plans, as well as supervising, instructing, and supporting care staff.

Qualifications
