CCH Group, the UK's largest and trusted care provider, is looking for a Care Coordinator for our Extra Care Scheme in Leeds. You’ll coordinate care plans, manage rotas, and support care workers to ensure high‑quality community care.

Location: Gascoigne house, Nightingale Court, Middleton, Leeds LS10 4YA. Salary: £25,364. Hours: Monday to Friday, 9am–5pm.

Qualifications

• Experience coordinating care for community-based clients.

Job Description

What you’ll do
The Care Coordinator plays a key role in ensuring the highest quality care services by managing care workers. Responsibilities include assessing and reviewing individual care plans, as well as supervising, instructing, and supporting care staff.

What you'll Bring:

Experience & Organisation: You are an efficient, organized Care Coordinator passionate about community care. You excel in managing rotas to impeccable standards and pride yourself on building strong relationships with care workers.
