Job summary

Hartwig Care Limited in London is seeking an experienced Care Coordinator to join our domiciliary care team. You will lead care staff, ensure timely client visits, and manage rosters with up-to-date information and policies.

This role requires strong leadership and collaboration with care managers and workers. The ideal candidate has at least 2 years in a similar role, solid scheduling skills, and excellent communication.

Qualifications

• Minimum 2 years as a Care Coordinator in Health & Social Care
• Experience with electronic care planning/rostering systems
• NVQ Level 3 in Health & Social Care or willing to work towards it
• Excellent people management and communication skills

Responsibilities

• Ensure care visits align with continuity and flexibility commitments
• Allocate visits to suitable workers based on client needs
• Provide up-to-date visit and policy information to the team
• Record information from care workers within agreed timelines
• Schedule care worker meetings and team reviews
• Cross-check visits against bookings for rota accuracy
• Support service monitoring, quality assurance and client safety
• Assist managers in communicating daily updates and changes
• Manage incoming calls and messages through the telephone system
