I’m delighted to be working with a small domiciliary care provider in Northampton who are now looking to welcome a Care Coordinator to their friendly team.

What’s on offer:

• £22,000-£24,000
• 28 days annual leave
• Additional on-call pay (receiving calls only)

Care Coordinator Responsibilities:

• Communicating regularly and professionally with clients and their families to ensure their care needs are being met and any concerns are promptly addressed.
• Liaising with pharmacies to ensure timely and accurate delivery of medication for service users.
• Supporting the Registered Manager with the completion of client assessments and risk assessments to maintain safe and person-centred care planning.
• Managing and scheduling staff rotas to ensure consistent and effective care delivery across the service.
• Responding to last-minute care changes or emergencies to minimise disruption to service users.
• Building strong relationships with care staff to promote high standards of performance and job satisfaction.

Care Coordinator Requirements:

• Experience working in a Care Coordinator rolefor a minimum of 12 months.
• UK Driving Licence and use of own car.
