Salary

• £25,545 Basic / £29K - £32,500 per annum with on-call
• Full Time, Permanent
• Monday - Friday, 8:30 - 17:00
• On-call one or two weeks per month @ £290 per week
• On-call once per month: £29K
• On-call twice per month: £32,500
• On-Call Hours: Monday - Thursday: 17:00 - 22:00; Friday 17:00 - Monday morning

Duties

As a Care Coordinator, you will play a pivotal role in scheduling and managing care rotas for our dedicated team of homecare workers. Key responsibilities include:

1. Planning and organizing rotas to ensure continuity and quality of care
2. Managing day-to-day scheduling changes and ensuring all shifts are covered
3. Liaising with clients and their families to arrange and confirm care schedules
4. Monitoring care plans, logging changes, and updating our Staff Plan system
5. Providing support and guidance to the care team, responding to queries, and handling emergencies when on call
6. Ensuring all regulatory and compliance standards are met in every care plan and visit

Criteria

• Minimum of 2 years’ experience in homecare
• Proven experience in care coordination and rota planning
• Car Driver Essential
• Strong organizational and prioritization skills
• Proficient with computer systems and scheduling software
