Job Overview

Leeds and York Partnership NHS Foundation Trust is recruiting for a Perinatal Referrals Management Service Care Coordinator to support regional mental health services for mothers and infants. This is a full-time, permanent position based at The Mount, Leeds, with flexible hours available.

Key Responsibilities
  • Coordinate referrals for the Yorkshire and Humber Mother and Baby Unit.
  • Work across community and inpatient services, liaising with mental health professionals and social care.
  • Attend CPA meetings and manage complex social circumstances sensitively.
  • Lead partner assessments and support early help plans.
Qualifications & Experience
  • Registered Mental Health Nurse, Occupational Therapist, or Social Worker with relevant experience.
  • Experience working with women in the perinatal period and understanding of safeguarding and diversity issues.
