Somerset NHS Foundation Trust is seeking a clinician to join the Single Point of Access Care Coordination Urgent Care Team. The role focuses on delivering evidence-based, patient-centred care and building practical skills in the speciality.

The post operates seven days a week from 08:00 to 20:30, with flexible hours on a rota and support from experienced Band 5 staff for Somerset Primary Link and Remote Monitoring. NHS pension and ongoing development are offered.
