Job summary

South West London & St George's Mental Health NHS Trust is recruiting a Band 6 Care Coordinator for the Richmond Integrated Recovery Hub. The post focuses on clozapine and depot management in the community and offers future development as a clinician within a multi-disciplinary team.

The successful candidate will monitor physical health, ensure safe venipuncture, and support patients aged 18–75 with varied mental health conditions in the London Borough of Richmond and surrounding areas.

Qualifications

• Experience monitoring clozapine and depot administration.
• Experience in physical health monitoring and Nursing/Medical procedures.
• Ability to work within a multidisciplinary team (MDT).

Responsibilities

• Take blood samples from patients ensuring consent is obtained before undertaking the procedure.
• Ensure blood samples are analysed and, where required, sent to the laboratory within specified time scales.
• Manage blood results and take actions when results fall outside normal parameters.
• Escalate concerns around patients' physical health to GP, MDT, and onsite pharmacist.
• Oversee clozapine applications and report issues to IT or managers when systems fail.
• Ensure venipuncture is performed safely and in line with infection control procedures.
• Assess general wellbeing of patients attending the clinic and discuss concerns with relevant practitioners.
• Provide physical and mental health monitoring and interventions in line with NICE guidelines.
• Maintain contact with pharmacy to ensure awareness of non-attendance and clinic stocks.

A great opportunity exists with a vacancy arising for the role of Care Coordinator in The Richmond Integrated Recovery Hub the We are open to applications from Community Psychiatric Nurses - tailored to clozapine and Depot administration. The service offers support to adults aged 18-75 with a range of mental health conditions.

The successful applicant will have scope to develop as a Clinician, in a multi-disciplinary team. Care Coordinators reports the Team Manager who reports to the Clinical Manager for Adult Community Services in the Borough of Richmond.

This Band 6 posts offers an opportunity for a passionate nurse who is versatile to support patients who are receiving clozapine and intramuscular (depot) medication in the community. If successful you will have overall responsibility in providing this essential clinic to patients within the London Borough of Merton.

This post makes a difference to our patients and provides the stability and continuity of care required. You will need to have experience of providing physical health expertise of monitoring side effects and demonstrating parity of esteem.

It also provides the opportunity for the development of students and colleagues in the administration and management of Clozapine and antipsychotics.

The post holder will:

• To take blood samples from patients ensuring consent is obtained before undertaking the procedure.
• Ensure that blood samples are analysed using the PocHi and where required sent to the laboratory within specified time scales.
• Managing blood results and take actions needed when results are outside normal parameters, following policy and national guidance.
• To escape concerns around service users' physical health to the GP, MDT, and onsite pharmacist.
• To escape any concerns regarding IT failure and or the running of the clinic.
• Managing clozapine applications like ZTAS and reporting issues to IT or managers when systems fail.
• Ensure venipuncture is undertaken safely and in line with Infection Control Procedures.
• Assess the general wellbeing of patients attending the clinic and discuss any concerns with relevant practitioners including pharmacy.
• Provide physical and mental health monitoring and interventions in line with NICE guidelines such as ECG's.
• Maintain regular contact with the pharmacy dept ensuring they are aware of patients who have not attended clinic.
• Ensure that the clinic is fully stocked with the necessary forms, consumables, sundries and equipment.
• Take responsibility for checking the cleaning, of all equipment and medications used in the clinic.
• Ensure all equipment is safe and in working order.
• Ensure that service users and carers are treated with dignity and respect.
• To provide teaching and training to student nurses allocated to the Community Team.
