Job summary

South West London and St George’s Mental Health NHS Trust is seeking a Care Coordinator (Band 6) in the Richmond Integrated Recovery Hub. The post supports adults with a range of mental health conditions in the community, with a focus on clozapine and depot administration.

You will work within a multi-disciplinary team, with opportunities to develop as a clinician and take overall responsibility for this clinic across the London Borough of Merton.

Qualifications

  • Experience as a nurse in adult community services or equivalent in multi‑disciplinary teams.
  • Experience with clozapine administration and depot antipsychotics in community settings.
  • Ability to monitor physical health, side effects and parity of esteem.

  • Provide care coordination in the Richmond Integrated Recovery Hub and across the London Borough of Merton.

Job description

A great opportunity exists with a vacancy arising for the role of Care Coordinator in The Richmond Integrated Recovery Hub the We are open to applications from Community Psychiatric Nurses – tailored to clozapine and Depot administration. The service offers support to adults aged 18-75 with a range of mental health conditions.


The successful applicant will have scope to develop as a Clinician, in a multi-disciplinary team. Care Coordinators reports the Team Manager who reports to the Clinical Manager for Adult Community Services in the Borough of Richmond.


This Band 6 posts offers an opportunity for a passionate nurse who is versatile to support patients who are receiving clozapine and intramuscular (depot) medication in the community. If successful you will have overall responsibility in providing this essential clinic to patients within the London Borough of Merton.


This post makes a difference to our patients and provides the stability and continuity of care required. You will need to have experience of providing physical health expertise of monitoring side effects and demonstrating parity of esteem.


It also provides the opportunity for the development of students and colleagues in the administration and management of Clozapine and antipsychotics.


Adhering to all relevant Trust policies and procedures and the Nursing and Midwifery Council Code of Professional Conduct in the delivery of this service.
