Job summary

We are looking to recruit a highly motivated and enthusiastic CPN/Care Coordinator to continue the excellent work in our well-established team based in Elizabeth Newton Building, Springfield Hospital, Tooting, London.

Wandsworth Intervention in Psychosis Service offers intensive support to clients who have experienced a first episode of psychosis and are between 18-65 years old. Evidence indicates that interventions at this crucial time can have a beneficial impact on the client and family, in many aspects of their lives. Using NICE-recommended interventions including CBT approaches, family work, and intensive support with an outreach ethos, the service offers a positive and recovery-focused approach.

Main duties of the job

The successful candidate will require previous experience of working as a care coordinator in the community and possess robust skills in all aspects of this role.

This post involves working autonomously with service users and their families, holding a caseload of approximately 18-20 clients (pro-rata). The Care Coordinator is responsible for planning, implementing, recording, and reviewing comprehensive care packages in conjunction with the multidisciplinary team. We particularly welcome applicants with psychosocial interventions training, including family interventions, and are willing to train all staff in this intervention.

Job responsibilities
  • Accountable for the coordination of care plans for individuals in early stages of psychotic illness aged 17-35.
  • Work closely with other multidisciplinary team members.
  • Assess health and social care needs and negotiate care plans with service users and carers, maintaining objectivity with diagnostic uncertainty.
  • Undertake risk assessments as per Trust policy.
  • Ensure delivery of care plans collaboratively within the CPA structure.
  • Engage service users and carers assertively, offering care/interventions within the least restrictive settings.
  • Provide a culturally sensitive service respecting age, culture, ethnicity, gender, sexuality, and disability.
  • Offer evidence-based psychosocial interventions and proactive risk management to reduce symptoms and support recovery.
  • Ensure understanding of Child Protection Guidelines and meet safety needs of children at all times.
  • Monitor treatment, support adherence, and address unwanted effects promptly.
  • Provide psycho-education on first episode psychosis to service users and families.
  • Address basic needs such as housing, income, and access to education or training.
  • Recognize developmental needs and support vocational/educational assessments and plans.
  • Work with other disciplines and agencies on co-morbid issues like substance misuse.
  • Engage service users and families in relapse prevention and crisis planning, coordinating interventions to prevent hospitalisation.
  • Deliver a flexible, responsive service outside normal hours when necessary.
  • If hospitalisation occurs, participate in joint inpatient reviews to facilitate transfer/discharge to the least restrictive environment.
  • Participate in developing early detection strategies and public awareness initiatives for first episode psychosis.
  • Work flexibly between 8 am and 8 pm, including occasional weekends.
  • Maintain professional standards and comply with relevant legislation and guidelines.
