We have an exciting opportunity for an Interim Care Brokerage and Placement Coordinator to join us!

This is a full-time, fixed-term role for up to 2 years with hybrid working, and a salary of £28,193 - £32,104 per annum.

This is an excellent opportunity for an enthusiastic Care Brokerage and Placement Coordinator to join our Brokerage team.

The Role:

We are looking for a self-motivated person to join our Adult Social Care Brokerage Team.

You will be working as part of a Care Brokerage Team but also can work autonomously when required.

Your role will involve:

  • Commissioning and procuring services for various client groups.
  • We are looking for someone who has experience of negotiation, particularly in relation to health and social care services.
  • You will be working jointly with Care Managers, Social Workers and Service Providers.
  • Setting up domiciliary Care Packages/Re-starts/Suspensions and ending care packages in addition Sourcing Nursing/Residential care providers.
  • Obtaining services (residential and non-residential) as indicated in a client’s support plan to meet their needs for social care services following an assessment.

What we are looking for:

  • You must have good communication and IT skills.
  • Experience of accurate data entry, analysis and reporting from spreadsheets and databases.
  • Work well within a team.
