Job summary

Essex Cares Ltd in Clacton-on-Sea is hiring a Care Assistant (part-time, on-site) to deliver reablement community care to clients recently discharged from hospital. You will help regain independence and support daily living tasks such as personal care, mobility and meal preparation in people’s own homes.

• Provide reablement support at clients' homes after hospital discharge.
• Assist with daily living tasks such as personal care, mobility and meal preparation.
• Record observations and report to supervisor for ongoing care planning.
