Job summary

The post holder will join our Cardiac Rehabilitation team as a trainee advanced practitioner, under supervision, developing skills in assessment, diagnosis and treatment within a defined scope of practice.

You will undertake workplace-based learning and university modules aimed at progressing to a Band 7 Cardiac Rehabilitation Advanced Nurse Practitioner, with travel across Conwy and Denbighshire counties.

Qualifications

• Nursing background in Cardiology.
• Willingness to work towards or complete Masters in advanced clinical practice.

Responsibilities

• Provide specialist Cardiac Rehabilitation knowledge, expertise and skills for patients and families.
• Work autonomously and as part of a multidisciplinary team to ensure a safe and effective pathway for cardiac patients.
