Responsibilities

• Deliver cardiac rehabilitation care within a multidisciplinary team.
• Provide specialist Cardiac Rehabilitation knowledge, expertise and skills.
• Lead on and implement cardiac rehabilitation interventions in Conway and Denbighshire with travel across counties.
• Support patient recovery and self-management through the pathway.
