Betsi Cadwaladr University Health Board (BCUHB) North Wales is seeking an enthusiastic Registered Practitioner to progress into a Band 6 role through a developmental fixed-term opportunity (Fixed Term until 31.03.2029).

You will work within a multidisciplinary team to support delivery of high-quality Community Rehabilitation Services across North Wales, building leadership, care coordination and specialist rehabilitation skills while coordinating care for service users within Mental Health and

Qualifications

• Registered practitioner in mental health.
• Experience in community rehabilitation settings.
• Welsh language ability desirable.

Responsibilities

• Hold a caseload and act as Care co-ordinator under the Mental Health (Wales) Measures Framework, delivering rehabilitation focused services.
• Support repatriation of service users back to their home area and provide intensive community rehabilitation.
• Work within multidisciplinary teams to develop leadership, care coordination and specialist rehabilitation skills.
