Job summary

JOBSTUDIO PTE LTD is seeking a Care Coordinator to support the Cardiac Department in coordinating patient care. You will work with cardiologists, nurses, allied health professionals, primary care providers and community partners to ensure coordinated and continuous care.

You will participate in patient assessment, care planning, treatment follow-up and education, while tracking clinical outcomes and maintaining accurate records.

Qualifications

• Bachelor degree in science or nursing is required.
• Minimum 3 years of healthcare experience.
• Strong verbal and written communication abilities.
• Excellent interpersonal and collaborative skills.
• Patient-centered, proactive, and capable of independent work.

Responsibilities

• Coordinate patient care with cardiology staff and primary care teams.
• Assess patients and enrol them into the Transitional Shared Care Programme.
• Develop follow-up plans and monitor clinical outcomes.
• Arrange follow-up appointments across clinics and GP networks.
• Maintain patient records and programme databases.
• Support clinical audits and quality improvement activities.
