Job summary

Croydon Health Services NHS Trust is seeking an experienced Macmillan Patient Support Officer (Band 4) to join the Macmillan Nursing Support Team. The role supports people affected by cancer through their pathway and beyond, navigating the health and social care system with emotional, practical and financial information and support.

The post holder will work flexibly across face-to-face, phone, email and text contact, with a comprehensive induction and ongoing training.

Qualifications

• Experience in providing emotional, practical and financial information and support to people affected by cancer.
• Ability to navigate the health and social care system and coordinate with clinical teams.

Responsibilities

• Support patients through the cancer pathway and beyond, providing personalised care.
• Coordinate with CNS and Macmillan teams to maximise delivery of cancer pathways.
• Interact with patients face to face, by phone, email or text and provide wellbeing support.
