Main duties of the job This role will work alongside and support the Mental Health (MH) pharmacists as part of the multi-disciplinary team (MDT) working within Primary Care and Community mental health services.Applications are encouraged from any area in pharmacy, as there will be opportunities for the development of clinical skills as well as the opportunity to undertake further training. Detailed job description and main responsibilities The successful applicant will work closely with our clinical teams and Pharmacists in providing a high-quality medicines optimisation service to clinical areas within LYPFT. Key areas of responsibility include:· Completion of medicines reconciliation· Ensuring timely supply of medicines· Delivering a patient focused medicines optimisation service to clinical areas.· Support the timely and smooth transfer of patient care at the point of discharge.· Participate in the training and supervision of new staff, trainees, and wider healthcare team.
