Job summary

Gloucestershire Health and Care NHS Foundation Trust is seeking a permanent Bed Management professional to support bed allocation across Mental Health and Learning Disability inpatient services. The role includes making key clinical decisions, coordinating placements, and ensuring timely discharges across 37.5 hours per week.

Responsibilities

• Support Bed Management Lead in bed allocation across MH and LD Inpatient Services in line with Trust procedures and national guidance.
• Be responsible for making key clinical decisions when operating independently.
• Identify most appropriate placements for patients.
• Manage and record Delayed Transfer of Care accurately.
• Ensure single sex accommodation standards are maintained and breaches defined.
• Maintain high patient experience in line with Trust values.
• Liaise with Senior Management, Adult Social Care and ICBs for authorisation of OOA placements, Complex PoC, NHSE Commissioned Beds and Secure Patient Transport.
• Act as expert to provide specialist advice on complex admission and discharge planning to ensure continuity of care.
• Develop protocols and propose policy or service changes in own area.
• Ensure patients are discharged when medically fit with minimal complications.
• Undertake designated project work as required.

This role is offered on a permanent basis. The hours are 37.5 hours per week.

• To be key in finding the most appropriate placement for patients.
• To be responsible for the accurate management and recording of Delayed Transfer of Care
• To ensure that single sex accommodation standards are maintained, clearly defining when a breach of the standards is required.
• To ensure that the patient experience is to a high standard and in line with Trust values.
• To actively liaise with Senior Management, Adult Social Care and ICB's in seeking authorisation for OOA placements, Complex PoC, NHSE Commissioned Beds and Secure Patient Transport.
• To act as an expert to provide specialist advice in relation to all issues involving complex admission and discharge planning to ensure the continuity of care for patients.
• Develop protocols for own work and propose policy or service changes in own area.
• To ensure that patients are discharged when medically fit with minimal complications to the process.
• To undertake designated project work as and when need arises.

• Responsible for Day-to-Day Management of MH and LD beds in Gloucestershire Health and Care NHS Foundation Trust.
• Ability to work 12 hours shifts across the 7 days a week service.
• To work across all MH and LD inpatient settings within the Trust.
• Communicate widely at all levels within and outside of the Trust, specifically; MH Inpatient and Urgent Care Services, ICB and ASC, as required in relation to complex admission and discharge.
• Anticipate the demand on all hospital resources in relation to bed management and co-ordinate accordingly.
• Oversee patient discharge practice and act as an expert advisor across all inpatient units in the Trust.
• Deliver designated project work directly related to key directorate agendas.
