• Ensure medication is stored and administered per Medication Policy.
• Record and log relevant information per reporting guidelines.

Job description

Support Workers are central to Praxis Care's mission of providing quality care to individuals. Each Support Worker is part of a dedicated team that delivers high-quality, reliable support services. At Praxis Care, we pride ourselves on maintaining a service where respect and dignity are paramount. All advertised salaries are based on 39 hours (FTE) and will be pro rata to contracted hours.

Responsibilities

• , Support and empower individuals to develop or improve their independent living skills as outlined in person-centred plans, which may include assistance with domestic or social skills and personal care.
• Collaborate closely with Team Leaders and managers to prepare and implement individual and group support programs.
• Work with other Support Workers and Team Leaders to promote a safe working and living environment, which may involve resolving conflicts or challenging behaviours using appropriate techniques and language.
• Ensure that medication is stored and administered in accordance with Praxis Care's Medication Policy, as necessary.
• Be familiar with all administrative requirements of the service, ensuring relevant information is accurately recorded and logged as per reporting guidelines.
• Stay informed about all policies and procedures in place within the service (including Health & Safety) and participate in related training sessions.
