The successful applicants will need to have excellent communication skills with an ability to provide clinically effective care within the District Nurse Team. The Central Division of Betsi Cadwaladr University Health Board delivers District Nursing Services across the counties of Conwy and Denbighshire, working in an integrated way with partner organizations to deliver high-quality, evidence-based care. The goal is to maintain patients in their own homes and communities, avert hospital admissions, empower patients towards self-management, and implement seamless, patient-centered care and treatment.
You must be able to travel between patients' homes, both within towns and rural areas, in a timely manner.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
