The idealcandidate will have experience in a primary care setting, preferably as aPractice Nurse and additional qualifications in Long Term Conditions such asCOPD, Asthma, Diabetes and CHD are desirable, as are contraception and travelservice.

Job responsibilities
Professional:
- Assessproblems presented opportunistically by patients.
- Providecontinuing care to patients across the practice population.
- Initiatelaboratory requests and collect specimens as required, interpret specifiedresults and act accordingly.
- Performholistic assessment of patients attending for ear irrigation.
- Providea holistic approach to wound management and implement wound care in line withcurrent evidence-based guidelines and locally agreed formulary.
- ECG
- Performholistic assessment of patients attending for cervical cytology smear.
- Adviseabout vaccinations and injections and ensure they are administered underrelevant policies, guidelines, and PGDs. Adhere to the national childhoodimmunisation programme.
- Assistin the provision of minor surgery.
- Recogniseand manage anaphylaxis according to current UK guidelines
- Beable to perform cardio-pulmonary resuscitation according to current UKguidelines.
- Obtainand document informed consent (either written or verbal).
- Ensureinfection control guidelines are maintained.
- Communicateeffectively with other members of the primary health care team.

Managerial:
- Assistpatients to identify their health needs.
- Maintainaccurate records in line with practice policy and NMC Guidelines usingappropriate computer software.
- Contributeto the assessment of service needs.
- Responsiblefor the maintenance of equipment and stock control: including refrigeration,vaccine ordering, single use equipment, general treatment room equipment andstock and emergency rugs and equipment.
- Contributea nursing level perspective to the Practice Development Plan.

Educational:
- Identifypersonal development and training needs in conjunction with the Nurse Managerand Practice Manager.

- Participatein formal tutorials and informal training of all treatment room trainees, GPtrainees and other allied health professionals in the practice
Managementof Chronic Disease:
- Todevelop knowledge base and progress in the management of Chronic Diseasepatients within the primary care setting.
- Diagnosis,monitoring and development of individual management plans, agreeing these asappropriate with the patients and other health professionals.
- Checkingcompliance with and adherence to appropriate treatments using a holisticpatient cantered approach.
- Identifyingthe impact of treatment and implementing or recommending changes asappropriate.
Researchprojects:
- Co-operate andparticipate as required in any research projects within the practice.
