Job summary

The Dudley Group NHS Foundation Trust is seeking an experienced Practice Nurse to join the community-based practice team, covering Chapel Street Surgery in Lye, Stourbridge, and High Oak Surgery at Brierley Hill Health and Social Care Centre.

You will deliver high‑quality, patient‑centred nursing care in accordance with NMC standards and local policies, while working autonomously within your scope and collaborating with a supportive multidisciplinary team.

Qualifications

• Experience and competence in children's immunisations.
• Experience in cervical screening.
• Experience in travel health and vaccines.
• Wound management skills and dressings.
• Understanding and working within QOF requirements.
• Chronic disease management experience is a bonus.

Responsibilities

• Deliver safe, evidence‑based nursing care in line with NMC standards and trust policies.
• Assess clinical needs, plan and deliver care, evaluate outcomes.
• Administer routine and travel immunisations and vaccinations.
• Provide travel health assessments and patient education on risks.
• Undertake cervical screening and manage related follow‑ups.
• Assess and manage wounds with appropriate dressings and referrals.
• Support management of long‑term conditions and promote self‑management.
• Escalate deteriorations promptly and maintain infection prevention standards.
• Participate in audits, quality improvements and CPD.

Job description

• Deliver safe, effective, evidence-based nursing care in accordance with Trust policies, NMC standards, and national clinical guidelines.
• Work autonomously within the scope of professional practice, recognising limitations and seeking advice or support when appropriate.
• Assess patients' clinical needs, plan and deliver appropriate care, and evaluate outcomes.
• Administer routine and travel immunisations and vaccinations in accordance with national guidance and local policies.
• Provide travel health assessments, advice and vaccinations, including patient education regarding travel-related health risks.
• Undertake cervical screening in accordance with national screening guidance and local protocols.
• Assess and manage wounds, providing appropriate dressings, treatment, ongoing review and referral when indicated.
• Support the management of long-term conditions (LTCs), promoting self-management, adherence to treatment and healthy lifestyle choices.
• Recognise clinical deterioration or situations outside personal competence and escalation appropriately.
• Maintain infection prevention and control standards, ensuring safe clinical practice and compliance with relevant policies.
• Participate in clinical audits, quality improvement initiatives and continuing professional development to maintain and enhance clinical competence.
• The postholder may be required to undertake home visitsWe are looking for an experienced and enthusiastic Practice Nurse to join our Practice Team, working across Chapel Street Surgery in Lye, Stourbridge, and High Oak Surgery at Brierley Hill Health and Social Care Centre.

Experience of EMIS Web would be desirable, although this is not essential as full training will be provided.

To be successful in this role, you will need experience and competence in:

• Wound management
• Understanding and working within QOF requirements

Experience in Chronic Disease Management would also be an advantage.
