Job summary

Circle Health Group's Lincoln Private Hospital is seeking a Clinical Pharmacist for bank shifts. The role involves providing cover during busy periods and supporting the Pharmacy Department with medication reviews and patient counselling.

You will work autonomously within a multidisciplinary team, with duties including safe prescribing, dose adjustments, and ensuring supply continuity. The role is compensated at up to £27 per hour, reflecting experience and qualifications.

Qualifications

• Must hold GPhC registration as Pharmacist.
• Experience in clinical hospital pharmacy preferred.
• Strong knowledge of therapeutics and guidelines.
• Excellent communication and clinical decision-making skills.

Responsibilities

• Conduct medication reviews for inpatients and outpatients.
• Provide tailored patient advice on treatment plans, side effects, and adherence.
• Support safe prescribing through reconciliation and dose adjustment.
• Review and authorise prescriptions per guidelines and formulary.
• Promote safe, cost-effective medicine use aligned with policies.
• Collaborate with procurement and pharmacy staff to ensure supply.
• Monitor and report medication incidents per MHRA guidance.
