Job description

We have an exciting opportunity for a Bank Registered Nurse Band 5 to join our South Wiltshire Locality Hospital at Home service. Our service is currently going through a positive period of transformation, making this a wonderful time to become part of a supportive, dynamic, and forward‑thinking team. The post will be based at either Amesbury, Wilton, or Salisbury, working Monday‑Sunday 8am–6pm (would consider shorter shifts).

As a Bank Band 5 Registered Nurse, you will be a valued member of the Clinical Team, working across organisational boundaries to bring together the strengths of both acute and community services. Our Step‑up team focuses on helping people avoid unnecessary hospital admissions, while our Step‑down team supports individuals to leave the acute setting safely and earlier when appropriate.

Responsibilities

In this role, you will join a group of caring, innovative colleagues who are dedicated to the wellbeing of the local community. Together, you will help ensure that those who use our services receive high‑quality, compassionate care that truly makes a difference.

You will work with a good degree of independence, carrying out tasks and responsibilities delegated by the lead clinician. You will be an important part of the team, helping to meet the needs of patients in community settings, initially focusing on those referred through the 2‑hour Urgent Community Response (UCR) and later supporting the development of broader referral pathways.

Working within a multidisciplinary team and across primary, secondary, acute, and social care services in Wiltshire, you will play a key role in coordinating specialist interventions for people who might otherwise require hospital care, or who need timely support to leave emergency departments safely.

You will take responsibility for your clinical caseload and help ensure smooth coordination of care throughout each patient’s journey.
