Are you passionate about transforming care for people living with MS?

We're looking for a driven and compassionate Band 7 nurse to lead MS community services across Dartford, Gravesham, and Swanley. Working as part of our well-established Community Neurological Rehabilitation Team, you'll provide expert assessments, manage a complex caseload, and be the strategic link between acute and community services.

This is a key leadership opportunity for a nurse who thrives in collaborative, patient-focused environments and wants to make a meaningful difference to those living with long-term neurological conditions.

Main duties of the job

• Deliver advanced nursing assessments and care planning for patients with MS
• Manage a complex caseload and provide symptom management and education
• Act as the named nurse and primary contact for community MS patients
• Work closely with Darent Valley Hospital and other stakeholders to ensure continuity of care
• Lead service development and contribute to audits and quality improvement
• Mentor colleagues and contribute to learning and development across the team
• Represent the service in strategic meetings and forums

Job responsibilities

As a Band 7 Community MS Nurse with HCRG Care Group, you'll be part of a supportive team delivering specialist care to people living with Multiple Sclerosis across Dartford, Gravesham, and Swanley. In return for your expertise and leadership, you'll receive:
