Are you passionate about transforming care for people living with MS?

We’re looking for a driven and compassionate Band 7 nurse to lead MS community services across Dartford, Gravesham, and Swanley. Working as part of our well-established Community Neurological Rehabilitation Team, you’ll provide expert assessments, manage a complex caseload, and be the strategic link between acute and community services.

This is a key leadership opportunity for a nurse who thrives in collaborative, patient-focused environments and wants to make a meaningful difference to those living with long-term neurological conditions.

Main Responsibility

Deliver advanced nursing assessments and care planning for patients with MS
Manage a complex caseload and provide symptom management and education
Act as the named nurse and primary contact for community MS patients
Work closely with Darent Valley Hospital and other stakeholders to ensure continuity of care
Lead service development and contribute to audits and quality improvement
Mentor colleagues and contribute to learning and development across the team
Represent the service in strategic meetings and forums

The Ideal Candidate

The ideal candidate will have / be:

Registered Nurse with current NMC registration
Experienced in managing long-term neurological conditions in a community or specialist setting
Confident in leading clinical care and mentoring others
Passionate about improving services and patient outcomes
Excellent communication and coordination skills
Full UK driving licence and ability to travel across Dartford, Gravesham, and Swanley

Package Description

As a Band 7 Community MS Nurse with HCRG Care Group, you’ll be part of a supportive team delivering specialist care to people living with Multiple Sclerosis across Dartford, Gravesham, and Swanley. In return for your expertise and leadership, you’ll receive:
