The post holder will be able to provide expert clinical expertise, operational management, to the Virtual Wards (Hospital @Home) Model within Birmingham Community Healthcare (BCHC) NHS Foundation Trust.
The purpose of the role is to work across the system in a variety of city-wide settings but not exclusive to the community and inpatient units, and to in-reach into secondary care, to identify and assess patients, to prevent avoidable hospital admissions and facilitate early supportive discharge. The work will be undertaken collaboratively with Secondary and Primary Care, and Community Services, including Urgent Care Response, Early Intervention Team (EICT), and Specialists Services. The post holder will work attend integrated multidisciplinary teams (MDT’s) to provide advice and support and agreed data to support the Virtual Ward (Hospital @Home) Model.
You will be joining a community team in the midst of one of the largest Health and Social Care transformations in the country, with exciting changes and developments, preventing avoidable hospital admissions, and facilitating early hospital discharge.
Create an environment where clients are able to be at the centre of their own care and decision making processes sensitive to the needs of the person. Respect the dignity, wishes and beliefs of the service users, involving them in shared decision making and obtaining their consent to treatment/referrals.
Act to safeguard the patient taking emergency action where necessary, reporting any potential or actual risks to the patient immediately and following the safeguarding policy. Take responsibility for ensuring risks to safeguarding are reported in a timely manner within the team. Work at times within the Virtual Ward Hub and act as the shift lead role, triaging new referrals and allocation to appropriate discipline within the teams; monitoring patient progress, leading on safety huddles, integrated MDT s and case reflections within the shift lead role.
